Medical Records

The Medical Records section transcribes, organizes, and maintains medical records for both The Family Medicine Center and the Specialty Medicine Center.

To Request a Medical Record
Patients, Insurance Companies, Attorneys, Government Agencies, etc:
Call or write the medical records department for information on obtaining records. The requestor must submit a valid authorization form signed and dated by the patient/legal representative. Certain restrictions and fees may apply.
